Merge - Appointment Reason

Entering the reason for the exam ensures that the patient is scheduled correctly. This is a required step when scheduling.
Note: Reason for exam should never equal the exam type. 

1. Screening Mammogram Reasons:
   • NO ORDER is required by the physician, select "Self-Requested Screening"
     
   • Physician REQUIRES ORDER for screening mammogram, select "Routine"
     
2. All Other Exam Reasons:
   • All other exams, select "Other Appointment Reason" and notate the reason in the Other field
   • Example: 
3. Patients who are requesting an exam with a specific Radiologist
   • Select that Radiologist from the Appointment Reason drop down list, include the appointment reason in the Other field
